Background Check Consent Form for Coaches

As a member of the Tennessee State Soccer Association, we require that all coaches complete this online Volunteer Disclosure Form in order to process a background check. For our recreational coaches, the form is used solely for a standard background check and not for drug testing. The completion of the Volunteer Disclosure Form and the passing of the background check are required before any coach can participate in Goodlettsville Soccer.

MTFC is a volunteer organization. Therefore, we are always seeking parents and/or coaches that want to make a difference in the lives of children. Without volunteers our program would not be able to function. We have a training session on March 17 for parents who will be coaching for the first time or would like some tips and advice on how to run a successful training session. Details will be posted to our site soon. Please contact Carol Spurlock (carol.spurlock007@comcast.net) if you are interested in coaching this season.

No Dogs Allowed

Dogs are not allowed on the soccer playing fields by authority of the Goodlettsville Parks and Recreation Department. For questions call 851-2241.
